We are pleased that you have arrived to our online document upload webpage. Use the "Send File(s) to*" dropdown list to select the Community Assistant and Head Start/ Early Head Start center you are uploading documents to.

All of the fields below with a red asterisk * are required and you will not be able to proceed without entering the required information.

Important:

- If you open the link on a computer, the file(s) will have to already be saved on a computer so that they can be accessed for uploading

REQUIRED DOCUMENTS FOR ELIGIBILITY DETERMINATION

- Applicant’s Birth Certificate or other Age Verification

- Income Verification (prior 12 months)

- Proof of Residence

- Proof of legal custody (if the child is in foster care or guardianship)

- Notice of Action or Passport to Services (if receivingCalWORKs Cash Aid [TANF])

- Proof of Supplemental Security Income (SSI), if applicable

- Homelessness verification (if applicable and available)

- Current IEP (Head Start) or IFSP (Early Head Start), if applicable

- Full-Time Employment or Full-Time School/Training Verification (to receive full-day prioritization)

REQUIRED DOCUMENTS FOR ENROLLMENT (if selected for a vacancy)

- Current and complete physical exam completed by a physician

- Applicant’s Immunization Records (must be up to date for age)

- Completed TB Assessment and/or TB Test Results

AGE ELIGIBILITY

- For Early Head Start Pregnant Women: Any age for expectant mothers

- For Early Head Start: Ages birth to under 3 years old (age verification required)

- For Head Start: Child must turn 3 years old by September 1st and not yet age eligible for Kindergarten (age verification required)

INCOME ELIGIBILITY

Family gross annual income must be at or below the mandated Poverty Guidelines. Children with a current Individualized Education Program (IEP) or a current Individualized Family Services Program (IFSP) may be eligible for an over-income waiver (limited to 10% of the program total enrollment).

CATEGORICAL ELIGIBILITY (if applicable)

A family is eligible in any of the following categories, regardless of total gross income:

- Applicant is placed in 24 hour out of home Foster Care

- Family is receiving ongoing CalWORKs Cash Aid

- A family member living with and supported by the child’s head of household is receiving Supplemental Security Income (SSI)

- A family is experiencing homelessness as defined by the McKinney-Vento Homeless Assistance Act

A Community Assistant will work with your directly to ensure the required documents are submitted.
