RCOE Migrant Head Start Document Upload

We are pleased that you have arrived to our online document upload webpage. Use the Document Recipient dropdown list to select the Community Assistant and Head Start/ Early Head Start center you are uploading documents to.

All of the fields below with a red asterisk * are required and you will not be able to proceed without entering the required information.

Important:

- If you open the link on a computer, the file(s) will have to already be saved on a computer so that they can be accessed for uploading

As reminder, below is a listing of the type documents required for eligibility determinations:

REQUIRED DOCUMENTS FOR ELIGIBILITY DETERMINATION

- Applicant’s Birth Certificate or other Age Verification

- Income Verification (prior 12 months or last year’s W-2) must have worked in agriculture

- For migrant Families proof of migrancy/mobility

- Proof of legal custody (if the child is in foster care or guardianship)

- Notice of Action or Passport to Services (if receiving CalWORKs Cash Aid [TANF])

- Proof of Supplemental Security Income (SSI), if applicable

- Homelessness verification (if applicable and available)

- Current IEP or IFSP, if applicable

REQUIRED DOCUMENTS FOR ENROLLMENT (if selected for a vacancy)

- Current and complete physical exam completed by a physician

- Applicant’s Immunization Records (must be up to date for age)

- Completed TB Risk Assessment and/or TB Test Results

AGE ELIGIBILITY

- For Early Head Start: Ages 6 weeks old to under 4 years old (age verification required)

- For Migrant Head Start: Ages 6 weeks old to 5 years old and not yet age eligible for Kindergarten (age verification required)

INCOME ELIGIBILITY

Family gross annual income must be at or below the mandated Poverty Guidelines. Children with a current Individualized Education Program (IEP) or a current Individualized Family Services Program (IFSP) may be eligible for an over-income waiver (limited to 10% of the program total enrollment).
